Statement of release of liability 

By booking an appointment and participating in lymphatic drainage therapy with Elite Wellness Studios, I hereby acknowledge and agree to the following:

1. **Assumption of Risk:** I understand that lymphatic drainage is a non-invasive therapeutic treatment designed to stimulate the body's lymphatic system. Although lymphatic drainage is generally considered safe, I recognize that individual results may vary, and there are potential risks, including but not limited to dizziness, soreness, or discomfort.

2. **Personal Health Responsibility:** I affirm that I have provided accurate and complete information about my medical history, including any known health conditions, allergies, or physical limitations. I agree to inform the therapist of any changes to my health before or during treatment. I understand that certain medical conditions may not be suitable for lymphatic drainage therapy, and it is my responsibility to consult with my physician prior to receiving treatment if I have any concerns.

3. **Release of Liability:** I, the undersigned, hereby release and hold harmless Elite Wellness Studios its employees, contractors, and affiliates from any and all liability, claims, demands, or causes of action for any injury, loss, or damages resulting from my participation in lymphatic drainage therapy or any related services.

4. **No Guarantee of Results:** I understand that while lymphatic drainage therapy has many potential benefits, results are not guaranteed and may vary from person to person.

5. **Confidentiality:** I understand that all personal health information shared with Elite Wellness Studios will be kept confidential and used solely for the purpose of providing my treatment.

By booking an appointment, I acknowledge that I have read and fully understand this release of liability. I am voluntarily participating in the therapy and agree to its terms.
